The purpose of maintaining a list of doctors, dentists and pharmacists who "have been disciplined" in the State of Oklahoma is to protect the public from poor quality health care that could lead to patient injury.

The respective Oklahoma boards documents actions taken on the doctors’ licenses. These records are available for inspection by the public. The information listed in the links above was taken from the files of the Oklahoma State Board of Medical Licensure and Supervision, The Oklahoma State Board of Osteopathic Examiners, The Board of Governors of Registered Dentists, and The Oklahoma State Board of Pharmacists.

The charges listed in each case is an allegation or finding which has led to an investigation and board action. The board is not a court of law and its decisions only affect the status of a doctor’s license and his or her ability to practice medicine legally in Oklahoma.

Unless otherwise noted, if probation is terminated or a license is reinstated, the license is considered to be in good standing and the physician/pharmacist accorded all the rights and privileges which go with it. Note: A period of probation may be "tolled" under certain circumstances. When a doctor/pharmacist leaves the state, the time on his probation stops. When he or she returns to Oklahoma to practice, his or her probation time picks up again. The town or state listed with each doctor/pharmacist represents the Board’s most recent address on file.
